Moving
No matter what TRICARE program you’re enrolled in, your benefit is portable, making it easy to transfer your coverage.
Moving Checklist:
- Do not disenroll from your TRICARE option before you move.
- Once you arrive at your new location, update your contact information with your provider, your regional TRICARE contractor and the Defense Enrollment Eligibility Reporting System (DEERS) as soon as possible.
- In Person: To add or delete family members, visit a local ID card office. Call the office first to verify location and business hours.
- By Phone: Call the Defense Manpower Data Center (DMDC) Support Office at 1-800-538-9552 to update your addresses, e-mail address and phone numbers. (TTY/TDD for the deaf: 1-866-363-2883)
- By Fax: Fax address, e-mail address or phone number changes to the DMDC Support Office at 1-831-655-8317.
- By Mail: Mail changes to the DMDC Support Office:

- Online: Visit the DEERS Web site and follow the steps to update your address, e-mail address and phone numbers online.
- Transfer your Prime coverage when you arrive at your new location. Call TriWest at 1-888-TRIWEST (874-9378) to find
out if TRICARE Prime or TRICARE Prime Remote (TPR) is available in your new city. If neither TRICARE Prime nor TPR are available,
you and your family will be covered under TRICARE Standard/Extra.
Moving Within Your TRICARE Region:
- Change your Primary Care Manager (PCM) by completing a TRICARE Prime Enrollment and PCM Change form for yourself and each dependent family member. You
only need to fill out the section regarding your PCM change. Send your completed form to your TRICARE contractor.
Moving to Another TRICARE Region:
When you move to a new TRICARE region, you'll transfer your enrollment to either TRICARE Prime or TRICARE Prime Remote depending on
where you live: in a Prime Service Area or a designated TRICARE Prime Remote location.
In either case:
- Submit a new TRICARE Prime Enrollment and PCM Change Form to your new regional contractor. The enrollment transfer is
effective for your whole family when your new regional contractor receives the form.
Note: If eligible members of your family reside in different TRICARE regions, the split enrollment feature allows you to enroll in TRICARE
