Changing Your PCM

Request a new Primary Care Manager (PCM) in two easy steps:

  1. Locate a new PCM from the online Provider Directory.
  2. Request a new PCM online through the Beneficiary Web Enrollment (BWE) site. To access BWE, you need a valid Common Access Card (CAC), myPay PIN or Family Member Account.

You may also print and complete a PCM Change Form and submit it to TriWest at the address noted on the form.
